Human Evidence
Large clinical development programs across type 2 diabetes, obesity, cardiovascular disease, chronic kidney disease, and MASH
Semaglutide has one of the most extensive clinical evidence bases among GLP 1 receptor agonists. FDA approved products include Ozempic and Wegovy, with oral semaglutide formulations also approved. The approved indication depends on the specific product and formulation. NCTR CRS
Important distinction: FDA approval of branded semaglutide drug products does not mean that every product containing or represented as semaglutide is FDA approved.
WHAT IS SEMAGLUTIDE?
Semaglutide is a synthetic peptide analog of human glucagon like peptide 1, or GLP 1, a naturally occurring incretin hormone involved in glucose regulation, appetite signaling, and gastrointestinal function.
It was engineered to retain GLP 1 receptor activity while remaining in circulation substantially longer than native GLP 1. Structural modifications and albumin binding contribute to a half life of approximately one week, enabling once weekly administration in approved injectable formulations.
Semaglutide has been studied extensively in people with type 2 diabetes and obesity, with additional large clinical programs examining cardiovascular, kidney, and liver outcomes.
MECHANISM OF ACTION
Semaglutide selectively activates the GLP 1 receptor.
BRAIN • APPETITE & SATIETY
GLP 1 receptor signaling in areas involved in appetite regulation can increase feelings of fullness and reduce energy intake. This is an important contributor to the reductions in body weight observed in clinical trials.
PANCREAS • GLUCOSE DEPENDENT INSULIN
Semaglutide stimulates insulin secretion when blood glucose is elevated.
Because this effect is glucose dependent, the insulin response diminishes as glucose concentrations fall. Semaglutide also reduces inappropriate glucagon secretion under hyperglycemic conditions.
STOMACH • GASTRIC FUNCTION
Semaglutide can delay gastric emptying. This can influence post meal glucose excursions and contributes to gastrointestinal effects associated with GLP 1 receptor agonists.
CARDIOMETABOLIC EFFECTS
Clinical outcomes research has demonstrated effects extending beyond glucose control and body weight. Specific FDA approved semaglutide products now carry indications involving cardiovascular and kidney outcomes in defined patient populations.
WHAT THE RESEARCH SHOWS
WEIGHT MANAGEMENT EVIDENCE
The STEP clinical trial program established a substantial human evidence base for semaglutide in obesity and overweight.
In the pivotal STEP 1 trial, 1,961 adults with obesity or overweight and at least one weight related condition, without diabetes, were randomized to semaglutide 2.4 mg or placebo alongside lifestyle intervention.
At 68 weeks, average body weight changed by:
Semaglutide 2.4 mg: −14.9%
Placebo: −2.4%
Approximately 86.4% of participants receiving semaglutide achieved at least 5% weight reduction, compared with 31.5% receiving placebo. New England Journal of Medicine
TYPE 2 DIABETES EVIDENCE
The SUSTAIN clinical development program evaluated injectable semaglutide extensively in type 2 diabetes.
Clinical trials demonstrated improvements in glycemic control and reductions in body weight. Cardiovascular outcomes research subsequently demonstrated cardiovascular benefit in appropriately selected patients with type 2 diabetes and established cardiovascular disease.
CARDIOVASCULAR OUTCOMES
The SELECT cardiovascular outcomes trial expanded the evidence base beyond people with diabetes.
In adults with established cardiovascular disease and overweight or obesity without diabetes, semaglutide reduced the risk of major adverse cardiovascular events compared with placebo.
This evidence contributed to an FDA approved cardiovascular risk reduction indication for Wegovy in the appropriate population.
KIDNEY OUTCOMES
The FLOW trial studied semaglutide in adults with type 2 diabetes and chronic kidney disease.
Ozempic reduced the incidence of a composite kidney and cardiovascular endpoint. FDA labeling now includes reduction of the risk of sustained eGFR decline, end stage kidney disease, and cardiovascular death in adults with type 2 diabetes and chronic kidney disease. FDA Access Data
LIVER DISEASE RESEARCH
Semaglutide has also been evaluated in metabolic dysfunction associated steatohepatitis, or MASH.
In 2025, FDA granted accelerated approval to Wegovy for adults with noncirrhotic MASH and moderate to advanced fibrosis. In the interim Phase 3 analysis supporting the approval, 63% of participants receiving Wegovy achieved MASH resolution without worsening fibrosis compared with 34% receiving placebo. Improvement in fibrosis without worsening MASH occurred in 37% versus 22%, respectively. Confirmatory outcomes research remains ongoing. U.S. Food and Drug Administration
IMPORTANT EVIDENCE LIMITATIONS
The evidence supporting semaglutide is unusually extensive for a peptide based therapy, but findings must still be interpreted according to the specific formulation, dose, population, indication, and clinical trial studied.
Results from an FDA approved branded formulation should not automatically be attributed to unapproved, compounded, or research grade semaglutide products.
Clinical trial averages also do not predict the response or safety outcome of an individual person.
SAFETY & RESEARCH CONSIDERATIONS
Semaglutide has been studied in large clinical development programs and has established FDA prescribing information. Its safety profile nevertheless includes important adverse effects, warnings, contraindications, and monitoring considerations.
COMMONLY REPORTED
Gastrointestinal effects are among the most frequently reported adverse reactions.
These can include:
Nausea
Diarrhea
Vomiting
Constipation
Abdominal pain
Dyspepsia
In STEP 1, gastrointestinal effects were commonly reported but were typically transient and mild to moderate in severity. New England Journal of Medicine
GALLBLADDER & PANCREATIC CONSIDERATIONS
Gallbladder disease, including cholelithiasis and cholecystitis, has been reported with semaglutide.
Acute pancreatitis has also been reported with GLP 1 receptor agonists. Persistent or severe abdominal symptoms warrant medical evaluation.
DIABETIC RETINOPATHY
Complications of diabetic retinopathy have been observed in clinical research, particularly in some people with pre existing retinopathy and substantial improvements in blood glucose.
Patients with a history of diabetic retinopathy may require appropriate clinical monitoring.
HYPOGLYCEMIA
Semaglutide's insulin stimulating effect is glucose dependent, but the risk of hypoglycemia can increase when it is used with medications such as insulin or insulin secretagogues.
THYROID C CELL TUMOR WARNING
FDA approved semaglutide products carry a boxed warning regarding the risk of thyroid C cell tumors.
Semaglutide caused thyroid C cell tumors in rodents. It is unknown whether semaglutide causes thyroid C cell tumors, including medullary thyroid carcinoma, in humans.
FDA labeling contraindicates use in people with a personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2.
REGULATORY CONTEXT
SEMAGLUTIDE IS AN FDA APPROVED ACTIVE INGREDIENT
This is an important distinction from investigational compounds such as Retatrutide.
Semaglutide is the active ingredient in several FDA approved prescription drug products.
OZEMPIC
FDA approved injectable semaglutide for defined uses involving adults with type 2 diabetes, including glycemic control and, in qualifying populations, cardiovascular and chronic kidney disease risk reduction. FDA Access Data
WEGOVY
FDA approved semaglutide formulations have indications involving chronic weight management in defined populations, cardiovascular risk reduction in certain adults with cardiovascular disease and overweight or obesity, and treatment of noncirrhotic MASH with moderate to advanced fibrosis in adults. A higher dose Wegovy injection was also approved in 2026 for weight loss and long term weight reduction in qualifying adults. U.S. Food and Drug Administration
ORAL SEMAGLUTIDE
FDA approved oral semaglutide products also exist, including Rybelsus and oral Wegovy formulations. NCTR CRS
FDA APPROVAL DOES NOT TRANSFER BETWEEN PRODUCTS
The regulatory status of the semaglutide molecule must be distinguished from the regulatory status of a particular product.
FDA approval applies to specific drug products, formulations, manufacturing controls, labeling, strengths, routes of administration, and indications.
A research grade or compounded product represented as containing semaglutide should therefore not be described as FDA approved simply because branded semaglutide medicines are approved.
FDA specifically states that compounded drugs are not FDA approved and are not reviewed by the agency for safety, effectiveness, or quality before marketing.
